Admissions & Registration

Southwestern Medical Center’s department of Patient Registration assists with a variety of check-in services including pre-authorization, admittance and scheduling.

Pre-authorization

Prior to admittance, we will submit and verify the patient’s current insurance information. If there are any difficulties with insurance approval or authorizations, we will notify patients prior to any medical procedures. For questions or concerns regarding pre-authorization please call 580.531.4735.

Registration

The following documentation is required when registering as a patient at Southwestern Medical Center.

  • Current insurance card(s)
  • Photo I.D.
  • Social security card
  • Advanced directive/Living Will (if applicable)

Access Your Patient Portal
